We work closely with other providers in Greensboro and the surrounding areas, both orthopaedic surgeons and other providers. If you have a patient you would like to refer to us we would be happy to help take care of them. Please see below for options regarding how to submit the referral.

1. Please call us directly at (336) 299-0099 to speak with one of our office staff. We will be happy to assist you to make the process as smooth as possible. If you have an urgent referral please call our office.

2. Complete and fax our Patient Referral Form with an attached patient demographic sheet and office notes. We will review the information to get the patient scheduled as soon as possible.

3. Lastly, you may click the button below to submit a request online. Please attach patient demographics form and clinic notes at the bottom of the form.
